Detransitioners: Their Testimonies, the Patterns They Share, and What They Reveal About Pediatric Gender Medicine

Detransitioners are people who underwent medical or social gender transition and subsequently reverted. They are not a hypothetical category: they are a growing, documented population who are beginning to speak publicly about what happened to them. Their testimonies share recurring patterns: childhood trauma, undiagnosed autism or mental health conditions, social contagion from peer groups or online communities, practitioners who fast-tracked them to irreversible interventions, and the suppression of studies that challenged the claimed benefits of transition. The cases include a whistleblower at Washington University's gender clinic, a researcher whose study showing puberty blockers don't help was buried by its own authors, and a detransitioner named Clementine who received blockers at 12, testosterone at 13, and a mastectomy at 14 — despite a documented history of childhood abuse.

The word "detransitioner" is new. The phenomenon is not. People who underwent surgical or hormonal interventions to appear as the opposite sex, and who subsequently stopped or reversed those interventions, have existed as long as the interventions have. What is new is that they are speaking publicly — and that their testimonies are being documented before the medical establishment can absorb them into a narrative of successful outcomes.

Clementine: Blockers at 12, Testosterone at 13, Mastectomy at 14

In October 2024, activist Billboard Chris (@BillboardChris) documented meeting a detransitioner he calls Clementine. Her treatment history:

  • Age 12: Prescribed puberty blockers by Dr. Johanna Olson-Kennedy, director of the Center for Transyouth Health and Development at Children's Hospital Los Angeles — the largest pediatric gender clinic in the United States
  • Age 13: Prescribed testosterone (cross-sex hormones)
  • Age 14: Double mastectomy (surgical removal of both breasts)

Clementine had a documented history of childhood abuse. Under standard psychiatric practice, a history of childhood trauma and abuse is considered a significant comorbidity that should trigger additional psychiatric evaluation before irreversible medical interventions. It did not.

The same Dr. Olson-Kennedy was the lead researcher on a study examining whether puberty blockers produce the psychological benefits claimed for them. The study found that they did not. The study was not published. Billboard Chris and investigative reporter Michael Shellenberger documented that Olson-Kennedy was aware of the negative results and had not disclosed them publicly — while continuing to prescribe puberty blockers at her clinic.

Jamie Reed: The Washington University Whistleblower

Jamie Reed was a case manager at Washington University's Transgender Center at St. Louis Children's Hospital from 2018 to 2022. She is married to a trans-identified man. She is not a conservative and did not come forward as a political opponent of trans medicine.

In February 2023 she submitted a complaint to the Missouri Attorney General describing what she had witnessed at the clinic:

  • Children as young as 8 years old being assessed for gender dysphoria
  • The vast majority of patients arriving with co-morbidities: depression, anxiety, ADHD, autism spectrum disorder
  • Practitioners ignoring or minimizing co-morbidities in the rush to affirm a trans identity
  • Patients who expressed regret or hesitation were not encouraged to slow down — their parents were coached to push through the hesitation
  • Staff who raised concerns were dismissed or marginalized
  • No long-term follow-up on patient outcomes

Reed was interviewed on video by journalist Kate Harris, describing in detail the disconnect between the clinic's public-facing narrative of careful, individualized care and the reality of fast-track affirmation regardless of patient history.

The Missouri AG investigation that followed Reed's complaint was among the earliest formal state investigations of a pediatric gender clinic in the United States.

The WPATH Standards of Care: Evidence Manipulation

WPATH — the World Professional Association for Transgender Health — publishes the Standards of Care (SOC) that most gender clinics worldwide claim to follow. SOC version 8 was published in 2022.

Journalist Jennifer Block documented, through obtained emails and documents, that WPATH leaders were directly involved in influencing — and in some cases blocking — systematic evidence reviews that were being conducted to determine whether puberty blockers and cross-sex hormones produce the benefits claimed for them. Specifically:

  • Age minimum recommendations that had been in earlier drafts of SOC 8 were removed before final publication
  • Evidence reviews commissioned by WPATH and NICE (UK's National Institute for Health and Care Excellence) both concluded that the evidence base for pediatric gender medicine was low quality or very low quality
  • The WPATH-commissioned evidence reviews were not published alongside SOC 8 — they were delayed, modified, and in some cases suppressed
  • WPATH leaders communicated with sympathetic researchers about how to neutralize findings they considered damaging

The WHO (World Health Organization) used WPATH SOC 8 as the basis for its own gender health guidelines — despite the evidence-manipulation that had been documented in the development of SOC 8.

The Cass Review: The UK's Landmark Reassessment

In April 2024, the NHS England published the Cass Review — a four-year systematic review of gender identity services for children and young people, led by Dr. Hilary Cass, a former president of the Royal College of Paediatrics and Child Health.

Key findings:

  • The evidence base for puberty blockers in children with gender dysphoria is remarkably weak
  • Most studies used to justify puberty blockers are of low quality and do not demonstrate the claimed benefits
  • The review found no good evidence that puberty blockers improve mental health outcomes
  • The review noted that puberty is not simply paused by blockers — it has developmental effects on the brain, bones, and reproductive system that may not be reversible
  • The GIDS (Gender Identity Development Service, the UK's main pediatric gender clinic) had been operating without adequate clinical governance or outcome tracking
  • Many young people presenting with gender dysphoria had significant co-morbidities that were not being adequately assessed

Following the Cass Review, NHS England announced that puberty blockers would no longer be routinely prescribed to children with gender dysphoria outside of research settings. The Tavistock clinic — the UK equivalent of Washington University's gender clinic — was closed.

The Cass Review is the most rigorous systematic assessment of the evidence base for pediatric gender medicine produced in any country. Its findings are consistent with the detransitioners' testimonies.

The Testimony Patterns

Across documented detransitioner testimonies — in court cases, interviews, social media accounts, and the MaryCate Delvey megathread compilation — several patterns recur:

Childhood trauma and abuse: A disproportionate number of detransitioners, including Clementine and Laura Becker, report histories of childhood sexual abuse, neglect, or family dysfunction. Becker's account: "I was abused by my father. Woke ideology told me my self-hatred was because I was trans. That was a lie. I was just an abused girl." Her account describes practitioners who identified her gender dysphoria as the primary presenting problem and fast-tracked her to affirmation — without addressing the abuse that was the documented root of her dissociation from her body.

Autism and neurodivergence: Studies of gender clinic populations consistently find autism spectrum disorder overrepresented. Autism is associated with rigid, identity-focused thinking and social difficulty — including social difficulty that can make an adolescent susceptible to an identity framework that explains their feeling of not fitting in.

Homophobia-driven transition: The testimony of a parent that she transitioned her son because he was feminine and gay — believing it was better to have a straight daughter than a gay son — is documented. This is the pattern that Lisa Marchiano, Helen Joyce, and other gender-critical clinicians and writers have described: gender-nonconforming, often same-sex-attracted children being funneled toward transition because their parents and clinicians are more comfortable with a trans straight child than a gay one.

Social contagion from peer groups: Detransitioners describe friendship groups in which multiple members simultaneously identified as trans, social media communities in which identifying as trans produced positive social responses, and the experience of being affirmed and celebrated for a trans identity in ways that other aspects of their personality did not produce.

No off-ramp: Multiple detransitioners describe trying to express doubt or hesitation and finding that clinicians and family members had been coached to treat any expression of doubt as a test to be supported through, not as evidence that the transition should be slowed or reconsidered.

Yarden Silveira: When the System Abandons Its Own Patients

The case of Yarden Silveira, documented by researcher Ψ Exulansic (@TTExulansic), describes a trans-identified person who died after surgeons refused to correct a botched neovagina (surgically constructed vaginal canal). Silveira had been banned from the hospital following disputes about care. The case illustrates the failure mode on the other end: patients who have experienced surgical complications from gender surgeries and who then find that the medical system that performed the procedures will not take responsibility for correcting them.

This pattern — procedures performed without adequate informed consent about complication rates, followed by institutional abandonment of patients who experience those complications — is documented in multiple cases and in the Jamie Reed whistleblower account.

What the detransitioner testimonies collectively document

  • Irreversible procedures performed on children without adequate assessment of co-morbidities
  • Practitioners who fast-tracked affirmation and suppressed doubt
  • Studies showing blockers don't help — buried by their own authors
  • Researchers who raised concerns — dismissed or excluded
  • The Cass Review (UK, 2024) confirming: the evidence base was always weak
  • Children with documented abuse histories receiving mastectomies at 14
  • Parents who were coached by gender clinics to override their own children's hesitation
  • A medical establishment that performed the procedures and is now, in many cases, refusing to help with the consequences
